Health insurance offsets the cost of treatment and management of illness. Whether an insurer is private or public the principle remains the same: the beneficiary agrees to pay a fixed premium, and in return, the insurance provider agrees to reimburse defined medical costs.

With the current escalation in medical treatment costs and number of treatment options, many insurance providers now insist that a manufacturer demonstrates a product's cost-effectiveness before agreeing to list it for reimbursement.
